Most schools can complete their roll return using their Student Management System (SMS).
If your school does not use an SMS use the template form below. Completed forms must be received by the data team no later than 8 September 2025. This return is for state, state-integrated and charter schools with students in year 9-15 only.
Please ensure you enter your school number on the front page and once completed, sign the declaration tab before sending – this is an audit requirement.
