Reading Recovery Forms

As part of the continuing evaluation of the implementation of Reading Recovery, schools with Reading Recovery are asked to provide information for the Ministry of Education on an annual basis. This information is used to examine trends in the delivery of Reading Recovery and the progress of students, and to assist the Ministry in developing policies regarding literacy.

Reading Recovery teachers, tutors and principals can become familiar with the online data collection by practising on this 'dummy' site. They can enter fictitious data, if they so wish. Tutors may also use this site for helping Reading Recovery teachers become familiar with the data collection process.

The final site is http://www.readingrecoverydatacollection.minedu.govt.nz/login.asp

This is the final site to enter 'real' data. All data should be entered into the online database by Friday 11 December 2009. Reading Recovery teachers, tutors, and principals can find the latest instructions on how to use the online database by downloading them in PDF format from the welcome page of the final website.

Paper Returns

For the 2009 data collection, we are asking Reading Recovery teachers, tutors, and principals to enter their Reading Recovery data electronically. Hardcopies will only be accepted for schools with limited IT facilities and other unforeseen circumstances. We ask Reading Recovery teachers of such schools to return their forms to their tutors by the above date, as has been done in previous years.

Hard copy forms can be accessed from the downloads below. To ensure that the forms print out within the required page size, they should be downloaded in PDF format.

Instructions for Reading Recovery Teachers 2009. This sheet is designed to assist Reading Recovery teachers in completing the forms.

School End-of-Year Report 2009. This form asks for school based information and is to be completed by schools operating Reading Recovery in 2009.

Individual Student Report 2009. This form is to be completed for each student who has been, or is currently in, Reading Recovery in your school this year (2009). This form can be completed throughout the year, as soon as a child discontinues from Reading Recovery, is referred on, or leaves the school.
